Description

The Nutritionist and Nurse Diabetes Prevention Program is a CDC-recognized, evidence-based PreventT2 program led by a licensed nurse with expertise in diabetes, community health and social determinants of care. The program addresses barriers such as food access, health literacy, transportation, and cultural foodways while supporting sustainable lifestyle change.

Participants receive one year of structured education, coaching, and group support designed to reduce risk factors for Type 2 diabetes. Research shows programs like this can lower diabetes risk by up to 58% through modest weight loss, improved nutrition, and increased physical activity.

The program is FREE for Medicare beneficiaries with affordable options for others. Services are delivered online with nurse-led coaching, digital tools, and care coordination. Serving participants in ALL U.S. states.
